Scale 5/6 NJC points 12-17, salary range £22,514- £27,514
35 per week
Contract is initially for 12 months (extension dependent on funding)
Leeds Irish Health and Homes is a community-based organisation offering a range of 1:1 and group work support for Irish people, people of Irish Heritage and the wider community.
This role will involve managing a caseload of service users, and working closely with LCH Neighbourhood teams in the areas of Chapeltown and Meanwood. We are looking to recruit two Health and Wellbeing Workers who will be working closely with LCH Neighbourhood teams in the areas of Chapeltown and Meanwood. This is an exciting opportunity to be part of a brand-new service (Enhance) responding to the needs of people being discharged from hospital, preventing readmissions, and promoting independence through person-centred support.
We require you to:
Work as part of the Enhance programme to develop ways to support people in their own homes and add to the joint working/partnerships approach of the Neighbourhood teams.
Provide client focussed work to empower individuals to engage with services and activities to improve their wellbeing.
Be responsible for collating information to meet monitoring and evaluation requirements.
We need you to have:
An outstanding attitude and track record in supporting vulnerable people.
Excellent organisational, time management and communication skills
