We are proud partners in Connect for Health Leeds – a friendly, local social prescribing service for people who live in South and East Leeds which provides access to local groups and activities that could help make a positive change to your life.
Connect for Health is a social prescribing service commissioned by Leeds South and East Clinical Commissioning Group and delivered by a consortium of organisations lead by Leeds Mind together with BARCA, Better Leeds Communities, HAMARA, Leeds Irish Health & Homes and Touchstone.
Social prescribing is an alternative to support through your GP when medical remedies may not be the right solution. This might be for instance when you are feeling depressed, isolated or lonely. A social prescription can help support you to find the right kind of service for you.
Connect for Health can help you work out what’s right for you and how you can access it easily. We understand that everyone is different, so we will work with you to find out what challenges you are facing and what you hope to achieve. We will then support you to access the right group or organisation.
